After creating a survey in Feedback Management, use the following sections to manage the data collection and export needs.

Adding a Survey Link to a Gnosis Email

Using Gnosis Communications, an email can be sent with a link to complete the survey online. When an online survey is completed, the results are immediately available in the person's record in addition to the full survey export. Follow the steps below to include the survey link in a Gnosis email design.

In the email editor of the desired item in Gnosis Communications, 

  1. Select the desired text or button to which you want to insert the survey link. This will display the URL field in the content editor on the right.
  2. Go to the Available Fields list > Feedback/Survey Fields > locate your desired survey.
  3. Each survey will have multiple field options. You may need to expand the fields list window to see the full name/options. Locate the URL option for the desired survey and double-click to copy.
  4. Go to the URL field in the email editor and paste the copied content (Ctrl-V or Right-click & choose Paste).

Completing or Viewing a Survey in a Person's Record

If using paper forms to collect survey responses, a Gnosis User may enter the information directly in the person's record on their behalf using the steps below.

  1. Open the person's record.
  2. Go to Feedback Response tab.
  3. Click New Form at the bottom and select the desired survey from the list.
  4. The survey will open in a separate window and it may take a few moments to load.
  5. Enter the responses and click Finish to save.

If wanting to review an already completed survey in a person's record, simply double-click the desired survey on the Feedback Response tab of their record. A separate window will open to view the saved responses.

 

Exporting Survey Results

  1. On the Feedback Management screen, select the survey from the drop-down list.
  2. Click Export.
  3. A Date Range prompt will appear: For all responses, leave the date fields blank. If desired, you can limit the export by the date of the responses using these date fields.
